Lake Park Lancer Booster Club Membership Form

To JOIN ON-LINE follow these simple steps:

Step 1 - Fill out the Join form below.

Step 2 - Proceed to the Booster Shopping Cart and join using your credit card by selecting the membership level that suits you. To see the various options, view the membership chart. Remember the $100 membership is our Best Value.

Step 1 - Print the Join Form and fill it out.

Step 2 - Mail your completed Join Form and Check with the membership dues payable to Lake Park Booster Club for the membership level you have selected. To see the various options, view the membership chart. Remember the $100 membership is our Best Value.

Booster Volunteer Information (Check all that apply)

Concession Stand Volunteer - Sell concessions during select home games
Logistics Contact - Pick up pre-arranged orders from Sam's Club and deliver to either East or West consession stands
Spirit Wear Volunteer - Sell spirit items during select home games/designated parent nights
School Store Volunteer - Sell school store items at either East or West Campus (requires lunch hour availability and a time frame of 2 1/2 hours)
Member Committee - Send out welcome kits to annual booster members
General Volunteer - Help out as needed at special events
Fund Raiser - Help out with various fund raising activities
Non-Active Member - I am not interested in volunteering - I am just happy to support the Lake Park Booster Club!

The Booster Membership Directory containing names and phone numbers of Booster Members will be an invaluable tool in locating teammate names and corresponding parent names to aid in carpooling between practices and games! In addition, it will help promote the communication between athletic parents. The directory is intended to be used responsibly and any use as a mailing and/or contact list is strictly prohibited.
